Support Services • Supports Coordination

Supports Coordination offers a variety of services to assist individuals in areas such as personal care, home support, transportation, accessible adaptations, equipment needs, medical, social, legal, and other support services regardless of the funding source.

Coordination of all services we offer include:

  • Assistance with home modifications
  • Assistance in obtaining durable medical equipment and supplies
  • Assistance/set-up of personal emergency response
  • Coordinate transportation needs
  • Evaluate and determine personal care needs, and initiate referrals to provider(s) of choice for necessary services
  • Advocacy
  • Coordination of assistance with waiver and non-waiver related resources (i.e. housing, utility concerns, county assistance, office dilemmas, issues with other providers of services received, medical issues involving physicians' offices, etc.)
  • Nursing home transition
  • Frequency of contact between service coordinator and consumer is decided upon jointly
  • Referrals for mental health, counseling, or other issues/concerns
  • All service coordinators are required to obtain a certain amount of training hours per year to ensure knowledge of community resources in order to better serve the people with whom we work
  • Serving individuals in most waiver programs ages 18 and up
